Required Paperwork
All athletes, at all grade levels are required to have a current physical on file with the Athletic Director each year. Physical must be dated after the second week of May to be good for the following school year. They must also have the TPS consent form, the sportsmanship and the Edison Transportation forms. An athlete is not permitted to practice or play in games without the physical and consent forms on file. Physicals are offered at the end of the school year for approximately $15 and then again at the beginning of the school year for $15 at various locations throughout the district.
